Eligibility for Medicare Bulk Billing Services

A patient must present with a chronic medical condition. A chronic medical condition is one that has been (or is likely to be) present for six months or longer, for example, asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions and stroke.

CDM ( chronic disease management) items are designed for patients who require a structured approach and to enable GPs and Practice Nurses to plan and coordinate the care of patients with complex conditions requiring ongoing care from a multidisciplinary team

NDIS Referrals

All NDIS referred patients will also require a medical clearance to exercise signed by their GP. This ensures that the safety guidelines for exercise participation are communicated to a patient's GP.
Please be sure to check that your client is eligible for Exercise Physiology ( not physiotherapy)
Your client will also need to be self managed or plan managed.
Invoices will need to be paid in full at the end of each month for NDIS clients to continue with participation.
Please list the following in the form below:

• Plan manager details
• Invoicing details
• Budget code to be used.
• Client availability ( group or individual)

Workcover Referrals

BrainFit works closely as part of a multidisciplinary team of physiotherapists, psychologists and other allied health professionals to assist injured workers return to work or transition to other pathways.
Workcover Clients include PTSD work related injury and musculoskeletal injury.
